This site has a very handy way to look up whether a state has unconstitutional disclaimer requirements.

Example: I chose to search on "advertisement disclosure" (sic - they mean disclaimer).
Alabama:
Advertisement disclosure: Are committees required to disclose their identity on print advertisements?
AL ST § 17-22A-12
Any paid political advertisement appearing in any print media or broadcast on any electronic media shall be clearly identified or marked as a paid advertisement. It shall be unlawful for any person, candidate, principal campaign committee or other political committee to broadcast, publish or circulate any campaign literature or political advertisement, without a notice appearing on the face or front page of any printed matter, or broadcast at the beginning or end of a radio or television spot, stating that the communication was a paid advertisement and giving the identification of the person, principal campaign committee or other political committee that paid for or otherwise authorized such communication.

AS 15.13.090
(a) All communications shall be clearly identified by the words "paid for by" followed by the name and address of the candidate, group, nongroup entity, or individual paying for the communication. In addition, candidates and groups may identify the name of their campaign chairperson.
(b) The provisions of (a) of this section do not apply when the communication
(1) is paid for by an individual acting independently of any group or nongroup entity and independently of any other individual.
(2) is made to influence the outcome of a ballot proposition as that term is defined by AS 15.13.065©; and
(3) is made for
(A) a billboard or sign; or
(B) printed material other than an advertisement made in a newspaper or other periodical.

A.R.S. § 16-912
A.R.S. § 16-912 (A)

A. A political committee that makes an expenditure for campaign literature or advertisements that expressly advocate the election or defeat of any candidate or that make any solicitation of contributions to any political committee shall be registered pursuant to this chapter at the time of distribution, placement or solicitation and shall include on the literature or advertisement the words "paid for by" followed by the name of the committee that appears on its statement of organization or five hundred dollar exemption statement. etc.
